What happens next?
Once we receive your request, a member of the reception team will contact you to discuss general appointment arrangements. Please do not include private medical details in this form.
From 1 October 2026: deposits
A deposit will be required to confirm your appointment. It forms part of the total cost and is deducted from the final amount due. Cancelling or rescheduling needs at least 24 hours’ notice. Reception will confirm the deposit that applies to your appointment.
