Once we receive your request, a member of the reception team will contact you to discuss general appointment arrangements. Please do not include private medical details in this form.
Important appointment information
- Appointments are strictly non-transferable and may only be attended by the person for whom the appointment was originally booked.
- All clients are required to bring a valid ID card or identification document to every appointment. This must be presented to reception upon arrival.
Deposits: 50% of the service fee
A deposit of 50% of the service fee is required to confirm your appointment. It forms part of the total cost and is deducted from the final amount due. Cancelling or rescheduling needs at least 24 hours’ notice.
